HIPPA Policy

At Pinnacle Counseling, we are committed to maintaining the privacy and confidentiality of your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). This law requires that we protect the privacy of your individually identifiable health information (also known as Protected Health Information or PHI) and give you notice of our legal duties and privacy practices with respect to your PHI.

• The Right to Access Your PHI: You have the right to inspect and obtain a copy of your health information that we maintain, with limited exceptions.

• The Right to Request an Amendment: If you believe that the information we have about you is incorrect or incomplete, you may request an amendment to your record.

• The Right to Request Restrictions: You may request limitations on how we use or disclose your PHI for treatment, payment, or healthcare operations. While we are not required to agree to all requests, we will comply with those we are legally able to honor.

• The Right to Confidential Communications: You may request that we communicate with you in a specific way (e.g., via phone or email) or at a specific location.

• The Right to an Accounting of Disclosures: You have the right to receive a list of certain disclosures we have made of your PHI.

 

How We May Use and Disclose Your PHI:

We may use or disclose your PHI for the following purposes:

• Treatment: To provide, coordinate, or manage your healthcare and related services.

• Payment: To obtain payment or reimbursement for services we provide to you.

• Healthcare Operations: For quality assessment, training, licensing, auditing, and administrative purposes.

 

We may also disclose your information when required by law, such as in cases of suspected abuse, threats of serious harm to self or others, or legal proceedings.

Please note that while we strive to protect all electronic communications, including emails and telehealth sessions, no system is 100% secure. By choosing to communicate with us via email or telehealth platforms, you acknowledge and accept these potential risks.
